Position: Skilled Nursing LPN Staff Nurse
Location: Albuquerque, NM
Position Overview:
A Skilled Nursing LPN (Licensed Practical Nurse) Staff Nurse provides direct nursing care to patients in a skilled nursing facility or long-term care setting. They work under the supervision of registered nurses (RNs) and are responsible for delivering quality patient care, ensuring patient comfort and safety, and assisting with the coordination of healthcare services.
Responsibilities:
Patient care and assessment: Provide direct patient care, including medication administration, wound care, monitoring vital signs, and conducting physical assessments. Collaborate with the healthcare team to develop and implement individualized care plans for patients.
Medication management: Administer medications, including oral, topical, and injectable medications, following established protocols and ensuring accurate documentation. Monitor and report medication effectiveness and any adverse reactions.
Documentation and charting: Maintain accurate and up-to-date patient records, including assessments, interventions, and outcomes. Document all relevant information in electronic medical records (EMR) or paper-based systems. Ensure compliance with documentation standards and regulations.
Patient education: Educate patients and their families about their conditions, medications, treatments, and self-care techniques. Provide instructions for managing chronic conditions, promoting wellness, and preventing complications.
Care coordination: Assist in coordinating healthcare services and interdisciplinary care plans for patients. Collaborate with physicians, RNs, therapists, and other healthcare professionals to ensure comprehensive and coordinated patient care.
Collaboration with the healthcare team: Communicate effectively with the healthcare team, including RNs, physicians, and support staff, to provide ongoing patient updates, discuss care plans, and address patient concerns. Participate in team meetings and contribute to the development of care strategies.
Adherence to regulations and policies: Follow all healthcare regulations, facility policies, and infection control protocols to maintain a safe and compliant environment. Adhere to professional standards of nursing practice and ethics.
