Responsible for coordination of patient/client referral to community resources to facilitate discharge of Homecare clients in a timely manner, and to facilitate problem solving of psychosocial issues impacting clients and the implications for provision of Homecare/Hospice services. Develops a holistic, multidisciplinary plan of care utilizing the job functions within each of the five core components of case management: Coordination and Service Delivery; Physical and Psychological Factors; Benefit Systems and Cost Benefit Analysis; Case Management Concepts; and Community Resources. Interacts with RN case managers, physicians and interdisciplinary team to determine appropriate Homecare/Hospice services for patients/clients based on regulatory guidelines for each line of service. Responsible for making daily SIA visits and working On Call rotation.
Masters Social Worker degree required
Current Masters Social Worker license for Kentucky and ability to obtain IL license. BLS certified. Certified Case Manager preferred.
