Job Description


The Population Health Consultant will working closely with our primary care providers (PCP’s) to own and execute on defined Value-Based Care program performance metrics and support for Privia’s at-risk patients within a defined region. They are the primary point of contact and accountable for practice success with our largest and most complex groups. They are responsible for setting the strategic direction of the relationship, delivering data to providers and staff, and following through on action plans.

Travel Required: Yes, 50% within region (driving distance)

Essential Job Duties:

  • Sets the annual goals and strategy for key PCP partners
  • Meets regularly with PCP partners to ensure the annual goals are being met
  • Works closely with care centers to close key quality measures and makes recommendations for improved documentation and workflows related to quality measures.
  • Collaborates with healthcare providers, administrative and other care center staff to succeed in population health goals
  • Assesses care centers’ needs, including, but not limited to, education, training, support and resources
  • Provides on- and off-site support to healthcare providers, front/back office and administrative staff, including, but not limited to: providing education and training to providers and staff; developing and maintaining resource materials, guides and process workflows; mining, scanning and uploading patient medical documents from EMRs; and patient outreach.
  • Accesses patient health information and documentation through the EMR and various payer portals; Engages in patient EMR chart reviews and “sprint work” to help close care gaps for a variety of payers and programs, as needed
  • Ensures that activities are in compliance with organizational and external standards
  • Other duties as assigned

Qualifications
  • High School diploma or general equivalency; Medical Assistant diploma preferred
  • Ability to drive to physician practices daily
  • 5+ years' of experience in a medical practice setting
  • Prior experience with Health Insurance Portability and Accountability Act (HIPAA), the Centers for Medicare and Medicaid Services (CMS), Healthcare Effectiveness Data and Information Set (HEDIS) measures and Medicare Stars measures
  • Knowledge of medical terminology and procedures, ICD-10 and CPT coding, insurance processes
  • Previous experience with electronic medical records (EMR), preferrably Athena
  • MS Office literacy including intermediate Excel skills
