I Accountability Objectives:
Performs and coordinates data collection and reporting for the various service lines utilizing a concurrent/retrospective chart review process. Utilizes judgement along with established criteria in data collection and analysis to assist in the development and coordination of quality improvement activities. Adheres to regulatory requirements and ensures compliance and accurate reporting process.
II Position Qualifications:
Minimum Education, Licensure / Certification and Experience Required.
A. Education
- Experience and knowledge typically obtained through work as a Registered Nurse.
B. Licensure / Certification
- If RN, current and valid nursing license to practice in the state of Michigan.
C. Special Skill / Aptitudes
- Excellent customer service orientation skills necessary in order to deal effectively with various levels of hospital personnel, outside customers and community groups.
- Knowledge of data extraction from the medical record and the use of technology data collection applications.
- Demonstrates initiative and motivation as well as the ability to work independently.
- Must possess understanding of computers and software (spreadsheet and database), such as Excel and Microsoft for Windows.
- Excellent assessment and analytic skills necessary for data collection, problem solving, data analysis, statistical analysis, and research methodology.
- Strong communication (written and verbal) and interpersonal skills in order to deal effectively with various levels of personnel. Understand the formal and informal structure of the hospital, outside customers and groups and operate proficiently in all.
D. Experience
- Five or more years of experience working in a health care environment working with medical records.
III Duties / Responsibilities:
- Assesses, monitors and evaluates patient responses to outcomes through concurrent and/or retrospective chart review.
- Accurately completes all data collection in a timely manner in adherence 2 to specific time frames.
- Participates in the ongoing design of data collection tools utilizing appropriate methodologies.
- Communicates pertinent information related to the auditing process and documentation requirements/trends to the appropriate Department/Unit Managers, so department/unit staff can improve on the completion of necessary data requirements.
- Works in collaboration with physicians, nursing staff and other department leaders to continuously improve care delivered to patients as documented by evidence-based outcome measures.
- Uses computer technology to keep accurate records to support the data collection and reporting process.
- Supports the appropriate service lines in the development and evaluation of the Quality Improvement Annual Plans and other quality improvement efforts as needed.
- Provides technical assistance to appropriate service lines performing quality activities including, but not limited to:
- Participating in development and design of improvement activities and standards.
- Participating in design of data collection tools and procedures, including planning, measuring and assessing the effect of improvement activities.
- Analyzing, collection and tabulation go appropriate primary and secondary sources of importation.
- Displaying data in a meaningful and useful format.
- Attending meetings to assist in the interpretation of quality improvements studies.
- Maintain working relationship with appropriate service line staff/physicians, Medical Records Department, BMC2 (if appropriate) and ACC (if appropriate) to keep abreast of trends/patterns as they relate to data collection and quality issues.

- Supports and conducts one's self in a manner consistent with customer service expectations.
- In accordance with unit or departmental practice, determines that appropriate charges have been entered for the correct patient, encounter, date/time of service, with any required modifiers. Make corrections as needed per charge capture policy/practice.
- In accordance with unit or departmental practice, actively participates in on-going education and communication regarding revenue management.
- In accordance with unit or departmental practice, assists with tracking and monitoring of equipment assigned to the unit. Requests services for maintaining equipment as needed.
- In accordance with unit or departmental practice, proficient with the electronic health records for documentation, assessment, and care management, performing these activities concurrently with provision of care throughout the shift.
