UW Primary Care (UWPC) is a network of 16-Community-Based Clinics throughout the Puget Sound region. The Lopez Island clinic is located among the NW's most beautiful scenery and offers a wide spectrum of primary care services, as well as secondary care for this more remote region.

UW PRIMARY CARE-LOPEZ ISLAND has an outstanding opportunity for a PART-TIME RN CARE MANAGER (RN-2). The day-shift schedule varies according to staffing needs.

POSITION SUMMARY

  • The Registered Nurse for Rural Clinics is responsible for triaging incoming patient calls, TE’s in Clinical Pool and walk-in patients, assessing needs, giving appropriate clinical options and facilitating requests to primary care providers, specialists, health care facilities and community resources. The registered nurse functions as a part of an interdisciplinary team in providing healthcare and patient education when appropriate, regarding immediate care advice and preventative health behaviors for patients seeking care.

  • This individual supports managing complex patients by implementing treatment plans and facilitating patient’s access to appropriate health care services and community resources. RN interacts regularly with providers and personnel from within the UWMC system and outside to include, but not limited to: Pharmacies, clinic/office/hospital personnel, laboratories and home care nurses to coordinate care of patients.

  • Position assists health care professionals in the examination and treatment of clinic patients, provides individual and group patient education, conducts basic diagnostic tests, provides follow-up for patient post-treatment care, and performs a variety of clerical/administrative functions, provides standard health messaging and education. Participates in clinic activities to promote effective problem solving and communication

    LOCATION
    This position will primarily support our Rural Primary Care clinic at Lopez Island.

    SAMPLE OF RESPONSIBILITIES

    Care Management
  • Applies professional skills and knowledge of established concepts, principles, and practice
    to provide care coordination to UWNC patients.
  • Under the direction of Physician or Advanced Practice Clinician, administers vaccines, and
    medications IV, PO, IM, SQ, Draws blood and processes specimens for offsite lab testing
  • Acts as a patient advocate through effective communication with patients, family, and
    other staff regarding the patients treatment, ordered tests, procedures, medications and
    hospitalizations.
  • Under direction of the Physician or Advanced Practice Clinician, administers various tests in
    CLIA- WAIVED lab, including glucose tests, OGTT, INR, Federal and non-federal drug
    screens, pregnancy tests, TB Tests and other essential lab testing.
  • Partners with primary care providers, case managers and other providers in implementing
    an interdisciplinary plan of care.
  • Applies nursing process to identify and prioritize healthcare triage interventions that are
    appropriate and accurate, effective and reduces the risk of adverse outcomes.
  • Follows policy to document call inquiries.
  • Answers calls in a timely manner.
  • Maintains confidentiality of all caller and personnel issues.
  • Interacts with callers and other staff in a polite and courteous manner.
  • Documents all call inquiries according to policy and procedure. Manages multiple priorities
    by listening, assessing, speaking, and transcribing patient information in computer.

  • REQUIREMENTS
    Associates Degree in Nursing
    Registered Nurse with years of nursing experience

    Registered Nurse License in the State of Washington
