Clovis Community Medical Center opened at its current location as a 109-bed hospital in 1988. Since then, there's been nothing but growth. Today, Clovis Community has 208 private patient rooms and another 144 under construction. We've added a $68 million freestanding Cancer Institute and other specialty programs and now perform more than 13,000 surgeries annually.
Responsibilities:
The RN Admissions, receives admission/discharge/transfer information from internal and external sources and facilitates patient placement in appropriate bed assignment. Ensures the information is communicated to affected areas in a timely manner. Responsible for various aspects of care coordination, that includes facilitating interdisciplinary care across the continuum and efficient patient throughput to the most appropriate level of care. Assists in identifying appropriate utilization of resources and ensuring services provided are in the appropriate setting. In collaboration with the Command Center, the RN Admissions will assist in identifying patients appropriate for inpatient or observation admission status. Oversees, coordinates, and communicates discharge planning services for patients through the healthcare continuum. Collaborates with the multidisciplinary team on discharge processes and serves as a liaison on community facilities and resources.
Qualifications:
Education
Associate's Degree in Nursing required
Bachelor's Degree in Nursing preferred
Experience
2 years of experience in an acute care setting required
3 years of experience in Registered Nursing across a wide spectrum of Inpatient and ED specialties preferred
Licenses and Certifications
RN - Current State of California Registered Nurse license required
BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
ACLS - Advanced Cardiac Life Support Certification required
