POSITION:
The Breast Cancer Screening Panel Navigator is a critical member of our team who is responsible for outreach and care coordination services as they relate to the UW Medicine Primary Care Breast Cancer Screening Initiatives and population health. They focus on closing care gaps in preventative screening for UW Medicine patients.
The Breast Cancer Screening Panel Navigator is a highly visible position that works directly with Providers and our central outreach team. They serve as the point-of-contact for assigned patient populations. Assignments will be based on defined sub-population groups where there are gaps in care, aligned with health equity priorities. They will provide concierge services to better coordinate patient care within the UW Medicine health system and partner/affiliate organizations. In communication with care teams, this individual will mitigate potential roadblocks and navigate patients for optimal care delivery, including providing support for planning, scheduling appointments and coordination of services.
The Breast Cancer Screening Panel Navigator will perform work in support of the program and under the general direction of the Assistant Director, Population Health, and UW Medicine Primary Care.
RESPONSIBILITIES:
This position works closely with other roles in the Panel Navigator team and responsibilities include:
Patient outreach, care navigation and coordination
Appointment scheduling
Referral coordination
Medical record activities
Other program support activities
REQUIREMENTS: High school graduation or equivalent AND Two years of Patient Services Specialist (PSS) experience OR Equivalent education/experience.
ADDITIONAL POSITION-SPECIFIC REQUIREMENTS:
Excellent communication skills to include verbal and written comprehension/expression.
High competency in emotional intelligence skills – ability to navigate conversations with patients and families in the most compassionate manner to result in an outcome that is right for patient’s health and meets programs goals.
Demonstrates good judgment and common sense.
Skill in operating personal computer and use of Office 365 tools.
Demonstrated ability and willingness to maintain competency as required for job title and the unit/area(s) of assignment.
Demonstrated ability and willingness to exhibit behaviors consistent with standards and principles for service excellence, performance improvement and organizational values.
BENEFICIAL EXPERIENCE:
Bachelor's Degree in a medically related or public relations field AND two years of experience in a tertiary patient care setting involving interaction with a variety of medical services, contact with patients and families, interaction with physicians, and proficiency in medical terminology preferred.
Proven skill in working with medical providers and wellness coaches preferred.
