Job Description

Essential Functions of the Job

Provides care and maintains awareness of total nursing needs of patients/families assigned to him/her and is responsible for seeing they are fulfilled.

Administers and supervises the patient plan of care for each hospice patient for whom he/she is responsible, according to Federal, JCAHO, and appropriate Standards of Nursing Practice as defined by the Hospice Program.

Notifies primary physician and/or on-call physician on an as needed basis regarding patient's condition and changes in the patient's plan of care.

Monitors and documents patient's reaction to symptom management and pain control.

Reports all medication errors, adverse drug reactions, fall, and any unusual occurrences by completing an Incident Form and documenting in Adverse Events in Allscripts.

. Arranges for requisition of equipment needed in home setting when appropriate, such as oxygen, as well as arranging for pickup of equipment in a timely manner.

Delivers/picks up Hospice medications and supplies in the home setting when appropriate.

Completes Home Health Aide Care Plan and provides instruction related to the plan of care to the Home Health Aide; documents supervisory visits of HHAs at the home site at least every two (2) weeks.

Coordinates nursing service with other disciplines including social services, pastoral/spiritual care, and volunteer services.

Participates in Interdisciplinary Team Conferences; presents and identifies problems for evaluation and recommendation and then helps to coordinate the plan of care which will best meet the patient/families’ needs.

Ensures that the Interdisciplinary Plan of Care is kept current/updated of any changes and that the physician is updated on a regular basis as changes occur.

Responsible for initiating Bereavement Assessment Form on assigned patients.

Consults with Patient/Family Care Coordinator - Home Care on problems/safety issues encountered in caring for patient/family.

. Assists in providing an optimal safe physical and emotional environment for both patient and family.

Teaches, supervises and counsels the patient and family members regarding the nursing care needs and other related problems of the patient.

Teaches and directs Home Health Aides and other allied health staff for whom he/she has assigned responsibility.

Maintains accurate, current and complete records on all assigned patients. Completes patient Recertification’s and tasks in a timely manner.

Adheres to and effectively communicates Hospice's philosophy, goals, and objectives to community health agencies and to the public.

Assists in the referral, intake and discharge process as necessary. Performs SOCs on his/her own patients and/ or assists in SOCs where needed.

Assists in orientation and development of nursing practicum students during clinical experience in the Hospice Program.

Is responsible for maintaining the appropriate insurance verification on file with the business office as well as submits completed mileage voucher in a timely basis.

Will work in a clinical role as on-call nurse on scheduled weekends and holidays according to rotation process.

Will work in a clinical role as on-call nurse after working hours on weekdays due to emergency or staff shortage.

Ensures that all patient information is held in strict confidence.

Works together with hospice inpatient RN when a patient is transferred into the facility or into the home hospice setting regarding medications, supplies, and equipment.

Ensures that when charting, only approved abbreviations found in the procedure manual are used.

PERFORMS OTHER RELATED DUTIES AS ASSIGNED


Educational Requirements

Registered Nurse, B.S.N. preferred. State of Hawaii RN license.

Experience

Minimum two (2) years’ previous experience. Acute/Oncology Nursing Experience.

Skills Required

Dexterity in handling equipment and instruments.

Must have valid driver's license.

Personal automobile available for business use.

Ability to adapt to changing situations on short notice and with ease.

Able to express self clearly and effectively in oral and written form.

Effective interpersonal, communication skills.

Desire to continually upgrade self in knowledge, skills, and professional conduct.

Good driving record, as evidenced by a current driver's abstract.

Current CPR certification.
