Job Summary: The Residential Direct Support Professional ("DSP") is responsible for a variety of duties related to the overall care of individuals with developmental disabilities who reside in the group home. Direct Support Professionals implement all aspects of medical and individual habilitation plan. Direct Support Professionals are required to perform all duties in accordance with the State of New Jersey regulations and ATTAIN & GAIN COUNSELING, LLC's policy and procedures.

b. Essential Duties and Responsibilities:

1. The DSP reports directly to the Resident Manager.

2. The DSP completes, demonstrates proficiency and maintains all required trainings (including College of Direct Support);

3. Attends house meetings;

4. Is responsible for medication administration and proper documentation thereof;

5. Monitors and assists individuals with all activities of daily living including but not limited to provision of direct counseling, implementation of each individual's treatment plan, assistant with personal care, medical, dietary, social, recreational, financial, habilitative, and other needs within the program.

6. Provides transportation to and from activities;

7. Assist individuals when entering and exiting vehicles while on community outings as well as entering and exiting the premises safely;

8. Supervises meal times;

9. Assists in the implementation of individual plans for individuals and behavioral plan goals;

10. Identifies specific behaviors which need to be enhanced or diminished;

11. Assists individuals in development of self-care and personal hygiene skills;

12. Works with the Resident Manager/Supervisor in keeping track of finances (individual funds, petty cash, deposits, keep track of receipts)

13. Plans, prepares and assists the individuals in the pursuit of leisure, recreational, social and community activities in and outside the residence;

14. Supervises individuals while out in the community, including making sure that individuals do not wander away, as well as assisting individuals in negotiating their surroundings;

15. Ensuring proper level of supervision during sleep based on the individual's needs

16. Works all assigned schedules, including arriving and departing on time, requesting time off in advance and immediately notifying your supervisor of changes to his/her assigned work schedule;

17. Must report to the supervisor in the event you are late or are unable to come to work

18. Log in when arriving and sign out when leaving and stay in the program and continue to work until relieved by the next scheduled shift; It is up to the DSP to make sure that there is minimum coverage in the program at all times particularly before they leave at the end of their shift. It is the responsibility of the DSP to report to their supervisor anytime there is not a minimum coverage in the program.

19. Ensures that the residence is maintained in a clean, safe manner including upkeep and general household care.

20. Complete and maintain paperwork for the individual (medical records, treatment records, incident reports, notes)

21. Check and re-order medication and supplies, as need.

22. Ensures appropriate level of supervision during mealtimes and preparing food in a manner that is consistent with the individual's dietary and feeding instructions.

23. Ensures each individuals' rights are protected;

24. Treats individuals with dignity and respect;

25. Provides support to other team members when individuals are having behavioral issues;

26. Reports unsafe conditions/ incidents to the Resident Manager/Residential Supervisor

27. Follows safety protocols to foster a safe environment.

28. To report to your supervisor any physical, psychological, or sexual abuse of an individual by any person internal or external. This includes neglect and the maltreatment of the individual. All allegations or information relating to the possible or actual abuse must be reported to the program supervisor or agency representative.

29. It is the agency's policy to investigate all allegations of physical, psychological or sexual abuse of an individual. It is required that that all employees, when asked participate in an investigation. This will include but not be limited to, participating in an interview(s) and providing written statements. The DSP shall cooperate with the Licensee (ATTAIN & GAIN) and Department of Human Services Staff in any inspection, inquiry or investigation.

30. Cooperate in requests for random and for cause drug testing throughout employment.

31. In order to meet the individuals and /or programmatic needs, from time to time, you may be required to work in a program other than the one which you have been assigned to on an on-going basis.

32. You are required to perform your responsibilities as they currently exist or may change in meeting the needs of the program, under the supervision of your supervisor.

Qualifications: Valid NJ Drivers' License with no more than five (5) points; I also understand that it is my responsibility to immediately notify my supervision if I receive any moving violations and to immediately provide my supervisor with any pertinent information which could affect my status as an agency approved driver. I understand that either falsifying or withholding any such relevant information could result in termination as it could seriously jeopardize the safety and well-being of our individuals.

High School Diploma (or Equivalent); computer proficient; clears all criminal background checks, drug tests, fingerprints. May not be disqualified by the Department of Human Services prior to hire. Any Applicant for a DSP position must agree to have his/her name checked against the Central Registry of Offenders Against Individuals with Developmental Disabilities and shall complete an application to have his/her name checked against the Child Abuse Registry Information (CARI).

Education and Experience: High School diploma or Equivalent and at least one (1) year experience working with individuals with developmental disabilities is preferred; ability to lift/push/pull 30 lbs.; Knowledge of equipment typically used by persons receiving services, including wheelchairs and adaptive devices. Three references required.
