Reports To: HCBS Manager, Southeastern Wisconsin
Status: Full-time (approx. 40 hours per week)
Pay: $40K - $50K per year

The Down Syndrome Association of Wisconsin is a statewide non-profit organization dedicated to advancing the lives of individuals with Down syndrome and related disabilities and their families through awareness, education, information, programs, services, and the exchange of ideas and experiences.
DSAW provides programs and services throughout the lifespan (Parent’s First Call for new and expectant parents, children’s services, adult services, vocational services, and aging services), as well as educational opportunities and events.
We have an immediate opening for a Senior Case Coordinator (HCBS) in Southeast Wisconsin. The Senior Case Coordinator will provide staff oversight and coordination of our Case Coordinators and direct services within home based services in Southeast Wisconsin. The Senior Case Coordinator will provide regular onsite training for coordinators during intakes, annual family meetings, and during sessions with participants. In addition, the Senior Case Coordinator will work with the HCBS Managers and Director of HCBS to grow our internal resource library. This individual will also have a caseload appropriate to other work duties within the role. Previous experience in relational de-escalation methods is preferred, but not required. The Senior Case Coordinator will work closely with the HCBS Manager, Southeast Wisconsin, to create and roll-out standard operating procedures.
The Senior Case Coordinator will be solely responsible for managing the waitlist of families needing direct services in Southeast Wisconsin. This includes hiring and training new Case Coordinators as needed, and assigning new cases to the appropriate case coordinator based on experience, location, etc.
In addition to overseeing the Case Coordinators and existing home based services, the Senior Case Coordinator will work closely with the HCBS Manager, Southeast Wisconsin, to expand DSAW services to families throughout Southeast Wisconsin. The Senior Case Coordinator should be very comfortable representing DSAW in the community at events and other public opportunities. Our ideal candidate is both a big-picture strategist and someone who is able to assist Case Coordinators with day-to-day details and problems.
The Senior Case Coordinator should have a professional background in case management and overseeing case managers. The Senior Case Coordinator should feel confident in their ability to ensure that quality case coordination occurs throughout Southeast Wisconsin. This will include regular shadows and real-time feedback of coordinators during intakes, meetings, and on-site trainings.
We are looking for someone who has experience working with individuals with disabilities. This person should have excellent organization and prioritization skills in order to assist with case and staff management. The Senior Case Coordinator must be a skilled communicator, empathetic listener, and excellent problem-solver. A positive track record of previous management experience is required for this role. Overall, the candidate must be a fast learner who is passionate about helping individuals with diverse abilities achieve their individual goals.
In order to complete this job well, the Senior Case Coordinator must be very comfortable with technology, and have the ability to quickly learn new systems, such as a new Case Management System. The Coordinator should have a thorough understanding of MS Office/G Suite products. Experience with a participant management system and a time tracking/scheduling system is a plus.

While the hours for this role overall are flexible, the majority of direct services take place between 9:00am and 6:00pm. Thus, we generally expect the Senior Case Coordinator to be available during these hours M-F.

Given that the Down Syndrome Association of Wisconsin is a non-profit organization that is not engaged in the production of goods or services, the Fair Labor Standards Act (FLSA) and Wisconsin overtime law does not apply to DSAW employees. Nonetheless, this is a salaried position budgeted at $$40K - $50K per year.
