To provide all case management support services for adolescent residents including intake verification and aftercare services. This position requires at least a High School Diploma and two years experience working with adolescents is a plus. Must be at least 21 years of age. In addition must have a skill set that enables them to learn about and understand people who are different from themselves, thereby becoming better able to serve them in their own communities.
· Verify information provided during intake as provided by residents or legal guardians.
· Assist with discharge planning and coordinate aftercare services.
· Assist with scheduling and transport to/from appointments.
· Assist with initial intakes and treatment planning.
· Filing and maintaining paper charts for clients.
· Attend meetings with Clinical Supervisor and Therapeutic staff on an as needed basis.
· Coordinate training activities at the direction of the Program Administrator.
· Attendance and participate in training activities.
· Attend weekly staff meetings.
· Weekly grocery shopping.
· Co-ordination and monitoring of on-line school.
· Complete other duties as assigned by Program Administrator or Therapeutic staff.
· Engage in ongoing cultural sensitivity training to better serve a diverse client base.
· Actively participate in all PQI training and activities as needed and determined by your supervisor.
· Other duties as assigned or needed.
Minimum qualifications: Must pass Idaho Department of Health and Welfare enhanced background check. Current Driver’s license, good driving record and ability to operate 12-person passenger van in a safe manner. Excellent organizational skills. To be self-motivated and operate within a team concept with minimal supervision. Ability to work with difficult clients. Strong communication skills. Good problem-solving skills. Ability to follow agency policies and procedures. Be reliable and dependable. Hours will be 7am-3:30pm Monday through Friday including Holidays. Ability to perform basic computer tasks.
